Provides advocacy and resources for older adults and persons with disabilities who lives in nursing homes, assisted living, and other licensed adult care homes as well as behavioral health services.
Eligibility: Older adults and persons with disabilities who live in nursing homes, assisted living, and other licensed adult care homes as well behavioral health services
Application Process: Can do walk-ins, but prefer to operate by appointment; call for information
