Mental Health Supported Housing

(Program)

Provides community support services, rental stipends, and recipient specific advocacy for individuals with a serious mental illness. Offered to ensure that individuals with serious mental illness may exercise their right to choose where they are going to live, taking into consideration their functional skills, range of affordable housing options available, and the services and resources a person requires to maintain their residence in the community.

Eligibility: Oswego County residents ages 18 years and older with a primary diagnosis of serious mental illness, experiencing substantial impairments in functioning due to the severity of the their clinical condition, and have very low to low income or receive SSI

Application Process: Call or go online (https://www.oco.org/mental-health) for information and services

Required Documentation: Serious Mental Illness (SMI) verification form completed by a doctor, RN, or social worker
