Prepares and delivers nutritional meals to seniors, homebound, and disabled individuals and those who are medically unable to shop for and prepare meals. Provides home visits, phone contact and referrals, as well as case assessments to seniors.

Eligibility: Seniors, homebound, and disabled individuals residing in Baldwinsville, Cicero, Mattydale, North Syracuse, Liverpool (south of John Glenn Boulevard), Brewerton (south of 81 bridge), and parts of Bridgeport

Application Process: Intake application required for all clients and must be completed prior to meal delivery; call 315-452-1402 to begin application process
