Eligibility: Families in Jefferson and Lewis counties needing health care

Application Process: Appointment required; call for information

Required Documentation: New patients need to bring: social security card, valid insurance card, and co-pay (if applicable)

Fees: Accepts private insurance, military insurances, Medicaid, Child Health Plus; sliding scale fee available for families without insurance
