Family Permanency Program (FPP)

(Program)

Provides support to adopted youth and their families, either via post-adoption, post-guardianship, and kinship. Works to improve family functioning and provides access to community-based services.

Eligibility: Post-adoptive, legal guardianship or formal or informal custody agreement families, with a youth birth to 21 years of age, who are struggling

Application Process: Call for information and services

Required Documentation: Legal adoption certificate and court documents
