Provides services for children with a serious emotional disturbance, developmental disability, or who are medically fragile. Offers Child & Family Treatment Support Services, Home & Community Based Services and Health Home Care Management.

Eligibility: Individuals from birth to 21 years who have experienced trauma or who have serious health or behavioral issues and have Medicaid or are Medicaid eligible; eligibility is determined on a case by case basis and varies by service type
